Quantitative: Collecting and analyzing hospital readmission rates before and after implementation to measure effectiveness.
Qualitative: Conducting interviews with patients and staff to explore their experiences, perceptions, and barriers to success.
Combining these data provides both measurable outcomes and rich insights into why the program works or doesn’t.
